Abstract
A 50-year-old HIV+ve patient, on HAART with CD4 count 69 cells/ul presented with counting finger close to face vision and IOP of 42 mmHg in right eye. Examination of right eye revealed microcystic corneal edema, granulomatous keratic precipitates, florid iris neovascularization, prominent anterior chamber reaction, vitritis, multiple patchy retinitis lesions involving posterior pole and peripheral retina, retinal haemorrhages and disc edema. Left eye examination was normal. A diagnosis of ARN with NVG was made and intravenous acyclovir with antiglaucoma treatment was started. Systemic evaluation revealed esophageal candidiasis precluding use of oral steroids. She received intracameral bevacizumab and simultaneous intravitreal dexamethasone. Right eye retinitis lesions showed gradual regression and IOP was controlled over next 4 weeks. This report highlights an uncommon presentation of ARN with NVG and demonstrates optimal anatomical outcome with a tailored treatment approach.
