Abstract
A 35yr old male presented with progressive dimness of vision in LE for 1month; BCVA (RE):6/9, (LE):FC at 3ft, red desaturation and sluggish pupillary reactions in LE. Fundus examination showed macular serous retinal detachment with an active choroiditis patch in LE; RE showed an old choroiditis scar superotemporally. USG (LE): choroidal thickening with retinal detachment, confirmed by OCT. FA (LE): active choroidal lesion with leakage. Systemic examination revealed non-specific cervical lymphadenitis. Other tests for infectious and rheumatic diseases were normal, except for positive QuantiFERON TB Gold test. The patient was treated with antitubercular drugs, with addition of oral steroids after 2wks. Resolution of detachment was noted, BCVA (RE):6/9, (LE):6/24 after 2months.
Thus, Ocular TB may present with serous retinal detachment; IGRA assays and clinical response to treatment support the diagnosis.
