Abstract

A young male presented with a BCVA of finger counting with normal AS following trauma. The fundus examination revealed resolving VH with pre-macular haemorrhage. At 3 months there was persisting pre-macular haemorrhage which was confirmed with SSOCT with a BCVA of 20/80. Patient underwent 25-G PPV with ILM peeling. At 4 weeks postop, BCVA OS improved to 20/20, with SSOCT showing disruption of ellipsoid zone [EZ] and interdigitation zone [IZ] corresponding to the area of resolved pre-macular haemorrhage seen in fundus CP and FAF.

The pre-macular haemorrhages are presumed to be harmful to the underlying retina if longstanding due to direct chemical effects of haemoglobin and its catabolites or due to mechanical effect of haemorrhage when contained in tight sub-ILM compartment.

OCT is an excellent tool to assess damage to underlying retina secondary to long standing pre-macular haemorrhage and disruption of EZ and IZ are markers for photoreceptor damage.
