Abstract
53 years female presented with headache and decreased vision in right eye since 1 week. No history of ocular trauma or prior ocular surgery. On examination pupils were ill sustained OU with vision of FC ½ m in OD and 6/24 in OS. Color vision in OS was normal and could not be checked in OD. Anterior segment OU was normal. Fundus OU showed optic disc edema with elevation of peripapillary retino-choroidal layer OD>OS. OCT OU showed choroidal thickening and irregular RPE. No evidence of neurological or dermatological involvement. Laboratory investigations were normal. CT scan was normal. Clinical diagnosis of Vogt-Koyanagi-Harada disease was made. Patient was treated with 3 doses of IV solumedrol followed by tapering doses of oral steroids. Patient showed immediate response to steroids with BCVA achieved in OD 6/18 and OS 6/6. Although, VKH disease presents with granulomatous uveitis or serous retinal detachments, optic disc edema may manifest early in the disease course.
