Abstract
>Purpose: To compare the predictive ability of the conventional (Gopalkrishna grade 1-5) classification versus the bespoke algorithm, in outcome of contracted socket management
Methods: Retrospective chart review of 47consecutive patients in 6 years who underwent contracted socket reconstruction.Fornix shortening, volume deficit and surface deficit were anatomic factors (AF) analysed Vs conventional classification
Results: AF were shallow fornix (100%).Volume loss 61% and 44% surface deficit. Repeat surgeries were equal in the lower and higher gradings ( p=0.84) . Final retention of prosthesis was better in higher (worse) grading ( p=0.07). Outcome correlated better with AF ; 91% good outcome with <3 AF, vs79% with 3 AF. 5 had adjuvant 5fluoro-uracil injections. Overall 85% achieved good cosmesis. Conclusion: Bespoke approach with adjuvant treatment gives good outcome in all grades of contracted socket. Anatomic factors help plan and predict better than conventional classification
