Abstract
A 67Y old lady presented with sudden loss of vision post trauma.VA was HMCF, IOP-22.Through old SICS tunnel PCIOL & iris was prolapsed into subconjunctival space.Clinically Ant chamber showed hyphema.Seidel’s test was negative.B-scan showed VH, retina was on.After initial medical Rx IOL explanation with iris repair & secondary IOL was planned.Under local anesthesia initial two partial scleral flaps were made at 3 & 9 clock.23G ports were made & plugged.Subconjunctival IOL was removed, healthy iris was reposited.Hyphema was cleared & almost 270 degrees of inferior fibrosed irido-capsular support was noted.SICS tunnel area debris was cleared & single stay suture was placed.PPV was completed to clear VH.Retina & periphery was examined.Initial attempt of placing same 3 piece IOL in sulcus failed & it dropped.After removing it, new 3 piece, mono-focal hydrophobic acrylic IOL was placed in sulcus with modified Yamane technique.Where haptic knobs were secured under partial thickness flap.
