Abstract
Hard cataracts , small pupils and compromised corneas remain challenging. Hard cataracts require specific strategies like torsional phaco and endothelial protection . Despite the advent of FLACS ,phaco and SICS remain the mainstay of hard cataract removal for 98 percent of ophthalmologists in India. Should we make the incision larger so that there is less wound burn, should we make a larger rhexis than recommended to ease nuclear management, what kind of device can we use to open small pupils ;certain cases are better managed with pupil hooks despite the advent of cheaply available pupil rings. Compromised corneas represent a sizeable portion of our cataract today.Especially in the elderly with harder nucleii, should we opt for DSAEK or PDEK at the time of surgery Should cataract surgeons be able to perform corneal surgeries themselves. Effective strategies to tackle these strategies will be taught in an interactive manner by the experts
