Abstract
An 84 year old man presented with discharge and maggots emanating from a cavity in the left orbit since 8 days.History of swelling on the left lower lid 4 years back which gradually extended to involve the left eye.Left eye had no perception of light. On examination an ulcerated cavity in the left orbit was noted with purulent discharge and destruction of the globe with uveal tissue partly intact. Maggots infesting the cavity were manually removed with forceps after application of Lignocaine and Turpentine oil.Systemic antibiotics and analgesics were started and dressing was done.Biopsy from lesion revealed Adenoid Cystic Basal Cell Carcinoma with secondary inflammation.CECT revealed soft tissue surrounding the left globe with intraconal extension medially and extending into the adjacent ethmoidal air cells with no evidence of intracranial spread.Systemic workup revealed no metastasis. Patient did not consent to exenteration of the orbit and reconstructive surgery as advised.
