Abstract
A 7 year old male child came to the paediatric ophthalmology unit of our institute with complaint of outward deviation of left eye since 8 months of age. There was no prior history of surgery, patching or spectacle use. Systemic examination was normal. His uncorrected visual acuity was 6/6 with insignificant refraction in both eyes. He adopted 45 degrees right face turn. On ocular examination the ocular movement showed limitation in abduction of left eye. On attempted adduction, narrowing of left palpebral fissure with upshoot was noted. A diagnosis of Left eye exotropic Duane’s Retraction Syndrome (DRS) with upshoot was made. Patient underwent asymmetrical bilateral Lateral Rectus Recession along with Y- Split in left eye. The aims of surgical management were to correct strabismus, centralise binocular vision and reduce need for compensatory head posture. At 1 month follow up, child showed significant improvement in head posture and upshoot and was advised right eye patching.
