A 68 years old female patient presented with history of headache and progressive loss of vision after a successful cataract surgery of right eye. She was a known case of esophgeal malignancy and had undergone esophagectomy one year back.VA in right eye was HMCF, whereas 6/12 in left eye.Anterior segments of both eyes were within normal limits. Fundus examination and OCT was suggestive of a choroidal mass lesion. A PET CT examination was done which showed a FDG avid lesion in eye. Diagnosis of choroidal metastasis was made and she was put on palliative chemotherapy. Her lesion responded to treatment initially and she regained some vision but she succumbed to recurrence in mediastinum. Choroidal metastasis following GI malignancy is a rare presentation and may be an early manifestation of recurrence elsewhere.
