Abstract
A 15 year adolescent male with complaint of persistent double vision ,2 weeks post repair of white eyed orbital blow out fracture .He reported to ED in unconscious state post head injury in road accident. Examination revealed left periorbital swelling with ecchymosis, marked tenderness to palpation over inferior orbital rim & infraorbital paraesthesia. He was suspected as a case of orbital floor fracture with entrapment of inferior rectus &herniation of orbital contents into maxillary sinus which was confirmed on CT scan. He was operated by oral facial surgeon for reduction of contents& repair of fracture using titanium mesh. He had persistent vertical diplopia with limitation of infraduction & negative force duction test . MRI confirmed stability of titanium mesh in the orbital wall thus excluding option of re-exploration, tear of inferior rectus was highlighted post orbital repair. The patient was managed conservatively and functional recovery was achieved 4 months after repair.
