Abstract
52 yrs old man, a known diabetic on irregular medication, brought by relatives with history of having non healing and neglected ulcer with large area of necrosis involving entire left upper eyelid skin of10 days duration. Patient on examination found to have maggots in periorbital region with non healing ulcer margins of the upper eyelid wound. Patient managed with irrigation with turpentine oil in and around the wound. 200 maggots were removed manualy and debribement of necrotic tissue from the wound was done. Patient also received single dose of Ivermectin (9mg, 200mcg/Kg). CT scan of orbit and brain revealed no intraorbital or intracranial involvement. Regular dressings were done for a week and later patient underwent skin grafting and wound repair at a higher centre.
Orbital myiasis is a rare ocular condition found in neglected wounds. Detailed examination of all ocular wounds with detailed systemic evaluation should be done to prevent serious and irreversible damage to the eye.
