Abstract
Result: A 47-year-old male, K/C/O diabetes for 1 year with h/o Covid 19 contact came with c/o fever, breathlessness, headache, diminution of vision in both eyes & left eyelid dropping with left eye swelling for 3 days. On examination DM & BP were uncontrolled. MRI brain with venogram suggestive of pansinusitis. Visual acuity in RE 5/60 and LE 1/60. LE findings clinically suggestive of third nerve palsy & orbital cellulitis. Patient was started on topical and injectable higher antibiotics with antihypertensive and antidiabetic therapy along with Covid management.
Conclusion: Third nerve palsy with orbital cellulitis in case of hypertensive crises & uncontrolled DM needs early diagnosis & aggressive treatment with coexist Covid infection as systemic co-morbidities in Covid are associated with high mortality.
