Abstract
PURPOSE: To describe outcomes of orbicularis oculi myectomy for refractory blepharospasm. This technique includes graded removal of orbicularis muscle, reattachment of levator,myotomy of procerus and corrugator.
METHODS:Retrospective chart review of 12 consecutive patients who underwent bilateral orbicularis oculi myectomy by a single surgeon. Outcome measures were change in botulinum toxin dose, frequency, and functional outcome.
RESULTS:
All (100%) who underwent orbicularis myectomy had significantly decreased botulinum toxin use with relief of symptoms postoperatively. 4/12 (33%) discontinued botulinum toxin. Interval between doses increased from 3 weeks to 14 weeks. Dose decreased from >100 units to 43units avg. 92% patients resumed activities of daily living. One developed hematoma and needed drainage. No patient had persistent lagophthalmos
CONCLUSIONS: Therapeutic myectomy technique provides effective treatment for refractory blepharospasm with good functional outcome
