Abstract
Reporting a case of 54 year old male,driver by occupation,known case of Diabetes Mellitus for 5 years and hypertension for 1 year on treatment,presented with complaints of eye ache R>L,giddiness and headache for 4 days.Headache was in the occipital area ,throbbing type.Initially treated elsewhere with analgesics and lubricants. This was followed by history of seeing objects smaller than they actually are and seeing distorted images since 1 day.On examination ,BP:200/100mmHg.Ophthalmological evaluation showed BCVA 6/9 RE and 6/9p LE. Anterior segment examination showed early lenticular changes in both eye. Fundus showed grade 1 Hypertensive Retinopathy BE. Perimetry revealed a left homonymous hemianopia.OCT macula was normal in both eyes. In view of the high BP and normal ocular examination findings, neurology reference was sought and MRI brain taken which showed acute/subacute infarct right occipital lobe.Patient was managed with antiplatelets and other supportive measures.
