Abstract
SYNOPSIS: A 26 year old female presented to the out patient department with the complaint of blurring of vision in the right eye noticed since 15 days. She gave history of cold, fever 15 days back. On examination, her distance vision was 6/6 in both eyes. Near vision in the right eye was N10 improving to N6 with +1.50DS and in the left eye was N6. The right pupil was 5.5mm in size with sluggish direct light reflex. Left pupil measured 2.5mm. This anisocoria was more in bright light (4mm).
Diluted Pilocarpine test (0.1%) was done. After 45 minutes, right pupil had constricted(3mm) while the left pupil remained the same. Provisional diagnosis of right eye Adie’s pupil was made.
CONCLUSIONS: This case stresses the importance of near visual acuity assessment irrespective of the age of the patient. Also meticulous examination of pupils must be done in all patients.
