Abstract
Blephrophimosis is characterised by bilateral congenital ptosis, epicanthus inversus ,telecanthus lateral ectropion of lower eye lid. Blepharophimosis correction required multi stage operation and prolong follow up. In 1st stage we correct epicanthus and telecanthus by muastarde double Z plasty and transnasal fixation by 1-0 prolene suture, which causes plication and close approximation of medial palpebral ligaments through nasal bone tunnel . Here 3.2mm manual orthopaedic drill is used for trans nasal tunnel in frontal process of maxilla at anterior lachrymal crest . Lateral ectropion is corrected by the skin flap based over zygoma and is transposed from upper eye lid to lower eye lid. After six months, In 2nd stage we performed lengthening of palpebral aperture by bilateral canthoplasty and at last moderated to severe ptosis corrected by bilateral silicon sling .
