Abstract
The purpose of this video is to detail the surgical steps, tips and tricks for retrieval of a lost muscle. Sixty-four-year female with aphakia and rhegmatogenous retinal detachment was taken up for pars plana vitrectomy, with 360-degree encirclage. After careful dissection of the intermuscular septa, all recti were bridled. Excessive intraoperative manoeuvrability by the retina fellow led to snapping of the medial rectus just behind its insertion. The transected medial rectus was retrieved by a strabismologist by dissecting along the orbital wall. The lost muscle was reinserted to the globe without causing significant heterotropia.
Retrieval of a lost muscle is a challenge. The surgical video overviews the surgeon factors that lead to muscle dehiscence during retinal surgery. Gentle and careful exposure of rectus muscles and preservation of muscle sheaths is important. Any muscle lost per-operatively should if possible be retrieved during surgery by a trained strabismologist.
