Abstract
Vitreous upthrust in Tripple Procedure ( OPK with ECCE and IOL) in young patients is very common and can cause havoc if not managed timely. We have innovated a new way of managing this unexpected problem by keeping 3 mm rim of trephinated recipient corneal button attached. So during cataract surgery, if vitreous upthrust is seen, one can temporarily suture the recipient cornea back and wait till upthrust is controlled by other means. When upthrust is controlled, cataract steps are done and then uncut 3 mm rim of trephinated recipient cornea is cut and donor cornea is sutured. So one can prevent dreaded complications like spontaneous lens or vitreous loss or even expulsive choroidal haemorrhage.
