A 61 year old male patient presented with a hypermature cataract with a patchy irregular anterior capsular fibrosis. Trypan blue was use to stain the anterior capsule. Zepto probe was used to create the continuous, curvilinear capsulorrhexis as there was a very high chance of the rhexis running out due to the fibrotic capsule. There was another membrane underneath the capsulorrhexis flap which itself was quite adherent. After a gentle dissection of the capsulotomy flap , the membrane underneath it was also dissected out & cut using microscissors & microforceps. Thus , now the dense, hard nucleus underneath the membrane was exposed , ready for phacoemulsification. A direct chop technique was used and the quadrants were emulsified carefully . There was hardly any cortex except for some thick cortical sheets in the periphery. We also dissected out a membrane underneath the peripheral anterior capsule. Patient was 6/9 unaided postop with a clear cornea and quiet eye.
