Abstract
A 12 year old girl presented to our OPD with complaints of gradual , painless diminusion of vision in her left eye followed by similar complaints in her right eye since the past 1 month. On evaluation bilateral ill sustained pupillary reaction, and mild blurring of nasal disc margin with mild hyperemia and a few telangiectatic vessels around the disc was seen in both the eyes. Based on her clinical findings a diagnosis of Leber Hereditary Optic neuropathy (LHON) was made and a trial of Intravenous methyl prednisolone was given following neurologist’s advice. As she did not have any improvement in her symptoms she was discharged with a guarded visual prognosis. This case has been presented for its extreme atypical presentation.
