Abstract
A 5 year old child diagnosed with Typhoid fever for 3 weeks was admitted in dept. of Pediatrics (treated elsewhere 1st) & presented to us with sudden painless loss of vision in both eyes for 2 days. O/E PL WAS DENIED IN B/E. Pupil –mid dilated, non reacting & fundus examination showed disc margin blurring, edema, hyperemia of disc B/E & OD hemorrhage in R/E.MRI showed no significant abnormality & her titre for ‘O’ & ‘H’ antigen were 1:320. Diagnosis of post typhoid immune mediated B/L Optic Neuritis was made .Treatment – she was on inj. Ceftriaxone for typhoid & was started on Methylprednisolone 25mg/kg/day iv for 3 days following which marked improvement was seen. BCVA on Day 4 was 4/60 B/E. Patient is currently on tapering steroid dose & on regular follow-up.
