Abstract
A 65 year old female presented with blunt trauma to her left eye with a tennis ball. The left eye showed 360° haemorrhagic chemosis with a well circumscribed, localised hematoma in superotemporal quadrant of size 1.5cm×1cm. Visual acuity was no perception of light. Patient had nil vision in that eye since last 30 years, details of which are not known. On slight pressure over the globe, leak noted at temporal aspect of swelling. CT orbit showed vitreous haemorrhage in left eye and could not comment on lens position. On wound exploration, cataractous lens was noted under subconjunctival space, superotemporally. After lens extraction, uveal tissue abscission and scleral tear suturing was done. Post operatively, patient received topical and systemic steroids and antibiotics and the patient was prognosticated regarding nil vision. Phacocele should be considered in cases of subconjunctival swelling with suspected occult scleral rupture and if the lens is not present in its natural position.
