Abstract
Phacoemulsification is one of routine procedure for cataract extraction in most parts of world but there is still skimpiness in developing countries like ours where MSICS is surpassing alternative for impoverished community surgeries.High postop astigmatism(SIA) accounts for significant cause of poor UCVA which can be prior determined by type&placement of incision to minimise preop existing elderly ATR astigmatism.METHOD:Ours is prospective randomised comparative study of total 300age related cataract cases divided randomly and equally into3 groups of superior,superotemporal and temporal scleral incisions where MSICS+PCIOL was done through unsutured 6.5 mm straight incision in 50 cases and frown in rest 50 in each group.RESULT: postop 4thweek 80%superior group had ATR astigmatism whereas 83.6%superotemporal&86%temporal had WTR astigmatism.Visual rehabilitation and decreased SIA were observed better in superotemporal and best in temporal group where84% cases also had postop UCVA of 6/9
