Abstract
Aim: Report outcome of 0.1% topical cyclosporine (Cs) in therapeutic penetrating keratoplasty(TPK) for fungal keratitis (FK). Method: Prospective interventional case series comparing 20 patients of FK treated with 0.1% Cs with a historical cohort of 28 patients of FK treated with topical 1% prednisolone acetate (Pa) after TPK. Cs was started on the day of surgery while Pa was started 3 weeks after TPK. Primary outcome: graft clarity at 3-months. Results: In Cs group 10(50%) grafts and in Pa group 4(14.3%) grafts remained clear(P=.011). In Cs group 1(5%) eye and in Pa group 5(17.9%) had fungal recurrence in graft (P=.379). LogMAR presenting visual acuity in Cs group 1.49+/-0.74 and in Pa group 2.10+/-0.62 (P=.003).Multivariate regression: Topical Cs independent factor for graft clarity(odds ratio:14.4(95%CI:2.4-8.6]P=.004). Conclusion: Early topical 0.1% Cs treatment is effective in maintaining graft clarity after TPK in FK, achieved better vision and was safe as recurrences were less
