Abstract
A case of S.marsescens endophthalmitis in hypertensive patient with post optical penetrating keratoplasty and past history of ocular trauma with metallic object is reported.Patient presented with complaints of diminution of vision in right Eye since 26 years following trauma.Ocular examination revealed pigmented leucomatous corneal opacity with peripheral thinning. Anterior chamber (AC) only seen from periphery which revealed PCIOL implanted in AC.Patient operated for penetrating keratoplasty with IOL explantation.On 1 st post operative day (POD), corneal graft had few descemet membrane folds along with 3+ cells in anterior chamber.On 8 POD, Culture report revealed S.marsescens.Patient was taken up for 23G Pars plana vitrectomy and intraocular foreign body was noted.On first day following IOFB removal, there was little graft edema along with 3+ AC reaction.He continued medications according to culture report.On subsequent visits, patient showed clinical and symptomatic improvement .
