Abstract
A 58-year-old male presented with metamorphopsia in the left eye since 1 month. His BCVA was 6/9, N8 in the left eye. The right eye was within normal limits. A swept-source optical coherence tomography angiography (SSOCT-A) revealed the presence of a type-1 (sub-RPE) CNV with underlying pachyvessel in the Haller’s layer, causing compression of the inner choroid and choriocapillaris. There was no definite macular edema, sub-retinal fluid or thickening. Patient was treated with three doses of monthly intravitreal injections of 2mg Aflibercept. The serial
SSOCT-A images performed at monthly follow-ups demonstrated regression of CNV with minimal but definite decrease in SFCT (from 609 to 565 microns). BCVA improved to 6/5, N6. Intravitreal Aflibercept seems to be a promising modality for treatment of symptomatic PNV, even in the absence of typical exudative features of associated CNVM. However, large scale prospective studies are need in this regard.
