Abstract
METHODOLOGY: 22 year old male presented with sudden onset paracentral scotoma of 15 days duration in OS. He was a chronic cigarette smoker. In OU BCVA was 6/6, colour vision and anterior segment were normal. Funduscopy revealed vascular tortuosity in OS and similar findings were seen in FFA with disc hyperfluorescence. OCT and OCTA findings in OS were consistent with PAMM. Coagulation profile was normal. A diagnosis of PAMM with impending vein occlusion was made and patient started on low tapering dose of oral prednisolone and pentoxifylline. At one month follow up patient showed reduction of scotoma confirmed on Amsler charting.
CONCLUSION: Owing to the rarity and subtle clinical findings, young healthy patients with this condition may sometimes be misdiagnosed as malingering on cursory examination. This report highlights the need for thorough evaluation in them.
