Abstract
The management of malignant glaucoma primarily involves core pars plana vitrectomy by the retinal surgeons or anterior vitrectomy with zonulectomy and iridectomy by the anterior segment surgeons. We are proposing a novel management strategy for malignant glaucoma which can be performed immediately at the outset of detection by an operating anterior segment surgeon and has shown a good success rate.In our technique visco-cohesive OVD was used for inflating the anterior chamber and synechiolysis through limbal route. Anterior vitrectomy with posterior capsulotomy was done using a vitrector through pars plana route made 3.5mm superotemporally using 23G trocar-cannula.An irrigation cannula on continuous mode was used to maintain the AC.Vitrectomy was continued till the deepening of AC was achieved.In our study of 9 eyes of 9 patients,all achieved resolution of malignant glaucoma with 8/9 eyes achieving significant IOP reduction (p=0.001) with vitrectomy alone in 8.6±6.1 months.
