Abstract
We report adverse local and systemic side effects/events following Intraarterial chemotherapy (IAC) in a large cohort of patients. Clinical data of 35 eyes undergoing IAC were reviewed and analyzed. Eighty-three of 89 successful catheterizations were done in 33 patients. Common arterial routes selected to deliver drugs were ophthalmic (34) and middle meningeal (13), giving a globe salvage rate of 77%. Major complications were local, regional, procedural, or anesthesia-related. These include CRAO(1), vascular occlusion(3), choroidal atrophy(12), vitreous hemorrhage (3), lid swelling (8), sectorial erythema on the skin(3), ophthalmic artery stenosis and spasm(5), lung collapse (2) and allergy to contrast (1). At a mean follow-up of 34 months, sequelae remaining were vascular occlusion related and choroidal atrophy, affecting vision. Intra-arterial chemotherapy gives high globe salvage rate but life-threatening complications and concerns of vision compromise remain.
