Abstract
This non-randomized prospective interventional study was undertaken to evaluate the surgical outcome of SR transposition (SRT) to LR with LR recession in the eso-DRS eye, with MR recession in the fellow eye. It comprised 10 patients of type III eso-DRS with co-contraction. Surgery improved primary position (PP) deviation from 23 PD to 6 PD; reduced abduction and adduction limitation from -2.1 to -1.1 and -1.7 to -1 respectively; improved face turn from 17° to 4°; and reduced globe retraction from 37% to 17%. Out of the 4 patients with shoots, none had shoots postoperatively. However, SRT was found to induce fundus intorsion and V pattern. Thus, a statistically favorable outcome (p <0.05) on PP deviation, abduction limitation, face turn, co-contraction and shoots was found. Poor candidates for this procedure include patients with V pattern or no pattern; and patients with pre-existing intorsion. Long-term outcomes need further elucidation by larger randomized controlled studies.
