A 26 year old male diagnosed with cervical spine anterolisthesis at C-6 and C-7 level. He had undergone a spinal surgical procedure following which he developed sudden loss of vision in his left within 4hrs of the procedure. His best corrected visual acuity in the right eye was 6/6 with normal appearing fundus, colour vision and fields. Visual acuity in the left eye was counting fingers at 1metre distance with opacification of posterior pole and cherry red spot,defective colour vision and fields. Fundus fluorescein angiography of right eye was normal and in left eye there was delay in filling of retinal vessels. He was diagnosed as a case of left central retinal artery occlusion. Electroretinogram of right eye was normal with left eye showing negative b wave. This case is reported for its rarity and also to ensure the efforts to be taken to prevent visual loss following spinal surgery.
