Abstract
Here we report case of a 64 year old male, presented with recent onset of diminution of vision in both eyes that had undergone cataract surgery in both eyes 6 years ago. Post surgery vision documented reveals 6/6 unaided in both the eyes. On examination patients refraction was -1.50DS in OD and -3DS in OS. RE showed adequate AC depth with well centred IOL in the bag, LE showed shallow AC with IOL in a distended capsular bag. However laser capsulotomy followed by laser PI failed to deepen the AC in LE. UBM showed no evidence of mass/effusion/rotation of ciliary body. Progressive shallowing of AC continued in both eyes with myopic shift and rise of IOP which could be controlled with topical AGM in RE whereas required trab+MMC+vitrectomy in left eye. After 12 years of cataract surgery currently patient has -4DS in RE and -4.50DS in LE. Progressive shallowing of AC with myopic shift following cataract surgery without any apparent cause is one of its first kind to be reported.
