To demonstrate the findings course and treatment in a patient of Pseudo Foster Kennedy Syndrome. My patient a 48yr old male presented with sudden painless loss of vision in Right Eye 5 days ago. Past history of sudden painless loss of vision in Left Eye 7 years ago. Vision RE counting fingers 1 metre LE BCVA 6/24. Afferent pupillary defect in BE. Fundus examination showed pale oedematous optic disc in RE with a splinter haemorrhage. LE had optic atrophy. FFA showed RE peri-papillary choroidal hypofluorescence in pre arterial phase, late phase had optic disc staining. LE had irregular optic disc fluorescence. HVF RE not possible, LE had inferior altitudinal field defect. Diagnosed as RE acute NA-AION LE Optic Atrophy due to old NA-AION. Patient was given high dose oral steroids tapered weekly. After 3 months BE had optic atrophy. RE vision was 6/36 with poor colour appreciation. LE had no change in vision. HVF showed RE inferior and central field defects. No improvement BE at 6 months.
