Abstract
A 20 Year old male reported with deviation in RE since birth and head tilt towards right side. RE Abduction restriction (-3) mild adduction restriction (-2) and mild narrowing of palpebral fissure on adduction present. LE – EOM full. Orthoptics examination showed RE exotropia 50 PD. WFDT – No BSV. His vision, anterior segment was normal. FDT – Positive for abduction, Negative for adduction. MRI Brain – normal. No evidence of paralytic cause. Patient didn’t have pre auricular skin tags , limbal dermoid, sensorineural hearing loss, vertebral anomalies. Systemic examination – normal. He was diagnosed to be Type 3 Duannes retraction syndrome . As patient had deviation in primary gaze with minimal globe retraction and compensatory head tilt with no overshoot or downshoot , patient underwent RE LR recession (9 mm). After surgery patient was orthophoric and compensatory head tilt also reduced .
