Abstract
Purpose: To study the mechanism of restoration of disorganization retinal inner layers (DRIL), after intravitreal bevacizumab (IVB) therapy in DME.
Methods: Forty-four patients aged 40-65 years having type 2 DM with DME were recruited for IVB comprising of three doses of bevacizumab (1.25mg in 0.05ml) at monthly intervals. Central sub-foveal thickness (CST) and grades of DRIL were assessed pre and post intervention on spectral domain optical coherence tomography (SD-OCT). Clinical trials registry: CTRI/2019/03/018135.
Results: Mean logMAR VA decreased after IVB regimen from 1.78±0.07 pre-intervention to 0.42±0.05 (p<0.001) post-intervention. Similarly, CST reduced from 354.23±15.0 µm to 233.18±7.88 µm (p<0.001). DRIL decreased from 93.2% cases pre-intervention to 86.4% (after first dose) to 31.8% (after second dose) to 13.6% (after third dose) (p<0.001). Conclusion: Anti-VEGF therapy improves DRIL with corresponding improvement in logMAR VA and decrease in CST in patients with DME.
