Abstract
Posterior reversible encephalopathy syndrome(PRES) is a clinico-radiological syndrome characterized by a headache, altered sensorium and visual loss. Characteristic magnetic resonance imaging(MRI) features include diffuse signal abnormalities involving the subcortical white matter in the parieto-occipital lobes. We describe a case of 18-year-old female with history of antepartum eclampsia who presented to our emergency room with complete loss of vision since last 2 days post lower segment caesarean section 3 days back. MRI showed evidence of bilateral symmetrical increased signal on T2WI and FLAIR sequence involving bilateral occipital lobes. These features were suggestive of PRES. The patient improved after conservative management with anti-hypertensive. PRES is associated with eclampsia and hypertensive encephalopathy. MRI is the gold standard for diagnosis of PRES. With high index of suspicion PRES can be recognized and treated early to reduce morbidity and hasten early recovery.
