Abstract
To evaluate the efficacy of oral doxycycline for matrix metalloproteinase inhibition role in treatment of fungal keratitis to prevent disease progression and corneal perforation.A prospective randomised controlled double blinded trial conducted in 363 patients who were diagnosed to have fungal keratitis. Patients randomly received oral doxycycline or placebo in addition to topical & Systemic anti fungal therapy.Size of ulcer, depth of stromal involvement & presence of hypopyon were noted at presentation and subsequent visits to evaluate for signs of healing.In 363 patients Group A -192 patients received oral doxycycline for a period of 2 wks and Group B-171 patients received oral placebo mimicking similar dosage and duration. Only 39 cases in doxycycline group required surgical intervention compared to 52 in placebo group which was statistically significant.Doxycycline group had only 4 corneal perforations when compared to 11 in placebo group which was statistically significant.
