Abstract
Post- operative endophthalmitis is a dreaded nightmare of every ophthalmologist and treating is a fierce battle to win especially when we are dealing with fastidious organisms which are difficult to isolate have broad spectrum of resistance and collateral damage due drug toxicity of the retina.Case Report: A 68 year old gentleman,1 month after uneventful cataract surgery in 1eye ,had features of endophthalmitis, with vision of 6/60.There was no improvement after 2 intravitreal injections of vancomycin, ceftazidime along with systemic and topical antibiotics and no yield from ocular samples or blood & urine culture.Hence vitrectomy with silicon oil implant was done. After improvement hypopyon kept recurring so re-vitrectomy with removal of IOL along with bag was done.samples sent for pcr & culture sensitivity showed Burkholderia cepacia which was sensitive to Cephazolin & Ceftriaxone. Infection was controlled with above antibiotics,with 5/60 vision in affected eye.
