Abstract
A premature male baby born at 28 weeks of gestational age and birth weight of 1100g had stormy neonatal course including systemic sepsis with Klebsiella pneumonia treated successfully with intravenous gentamycin. The 4th screening for retinopathy of prematurity (ROP) at post conception age of 33 weeks revealed right eye hazy cornea, absence of red reflex and no view of the fundus and left eye ROP stage 2 in zone 2. Ultrasound B examination of RE showed multiple vitreous opacities consistent with diagnosis of endophthalmitis. The baby was started on systemic antibiotics and was given intravitreal injection of 40 mcg of gentamicin in 0.025 ml presuming it to be gram negative organism. Vitreal tap was dry but the blood culture grew Streptococci agalactiae which is rare organism. The corneal haze improved but the vitreal haze did not improve. The parents refused further intervention. The aim of this paper is to highlight endogenous endophthalmitis caused by rare organism.
