Abstract
PurposeTo study pre & postop regional biomechanical change-keratoconic(KC)eye after crosslinking (KXL),topoguided removal of epithelium in KC (TREK) and topography guided surgery (TCAT) withKXL.Methods17, 54 and 26 eyes underwent KXL,TCAT and TREK & analyzed with corvis ST(CST)for regional biomechanics atpreand 6months postop.CST measures corneal zones superior,central and inferiorly.Linear&non-linear corneal stiffness derived from CST deformation waveformwith analytical biomechanical model.ResultsKXL and TREK showed increase in corneal stiffness in all zones.Central zone change-2.52±3.65 and 1.08±2.94 N/mfor KXL and TREK.TCAT-decrease in corneal stiffness in all zones,with significant change centrally (-3.78±2.04).TCAT achieved most manifest spherical equivalent decrease(39.8%),TREK(15.6%) and KXL(0.2%).ConclusionIncreased stiffness in TREK,being ablation driven,improved tissue specific modulus to achieve better corneal stiffness with manual KXL.No stiffness loss noted post TCAT+ KXL
