Abstract
Purpose: To report an interesting case of Urrets Zavalia syndrome(UZV) secondary to a high vault of Implantable collamer lens (ICL), after an uneventful ICL implantation
Methods: A 27 year old female presented intense photophobia and glare following an ICL surgery done elsewhere 3 months ago. Left eye had a very high vault and fixed dilated pupil along with a pupillary capture of the ICL
Results: After thorough evaluation and ruling out the ICL sizing error, the pupillary capture was relieved and the ICL was rotated into vertical position. Single pass four throw pupilloplasty was performed for the fixed dilated pupil. Post operatively the ICL vault and the pupil size decreased with improvement of the complaints.
Conclusion: Rotation of the ICL and pupilloplasty can be performed in cases of UZV after an ICL with good results. This is a first such case described in literature where a pupilloplasty in the presence of an ICL was done.
