Abstract
A randomised clinical study was done on 30 eyes with smear or culture positive fungal
keratitis, based on the severity of ulcer and not responding to topical natamycin for 2 weeks.
They were randomised to receive either topical voriconazole 1%(n=15) or intrastromal
injections of voriconazole 50ug/0.1ml(n=15). Topical natamycin 5% was continued in both
groups. They were followed up for 12 weeks.
Mean age of patients was 47±11.5years. Both groups had comparable baseline parameters.
The mean BCVA at 12 weeks follow up was 0.99± 0.17logMAR in topical group and 0.81±
0.15logMAR in intrastromal group, the difference being insignificant statistically. The mean
size of the ulcer was 2.45±0.79mm in topical and 2.18±1.00mm in intrastromal group which
was statistically significant(P<0.05). Ulcer healed in 53.33% in topical group as compared to 93.33% in intrastromal group(P<0.05). In conclusion, Moderate/ severe ulcers responded better to Targeted voriconazole than topical voriconazole.
