VT0188 – Successful Management of Large Giant Retinal Tear with Retinal Detachment – A Case Report

Abstract
We report the case of a 49-year-old male presented with acute loss of vision in his right eye. Fundus examination showed a 270 degree giant retinal tear (GRT) with large inverted flap and retinal detachment with macula-off. The BCVA was only logMAR 3.0. We performed 23-gauge pars plana vitrectomy and with the help of perfluorocarbon liquid (PFCL), we unfolded the posterior flap of the GRT, and cut the residual rolled borders, to better attach the retina. Then, we performed 360degree endolaser and silicon oil/PFCL direct exchange. We prescribe face down positioning in the post-operative period. The next day, the retina was completely attached and no folding of the tear borders was found. Primary microincision vitrectomy using wide-angle viewing system with intraoperative PFCL assistant and intraocular silicone oil tamponade appears to be safe and feasible for managing giant retinal tear with retinal detachment.
